From oldest to the newest they go like this: FUT (follicular unit transplantation, sometimes called FUSS for follicular unit strip surgery) FUE (follicular unit extraction) and DHI (direct hair implantation).  FUE and DHI have roughly the same extraction process; the difference comes in the implantation part. The Difference Between the DHI and the FUE Methods

The DHI and the FUE methods are very similar fundamentally. But it is safe to say that the DHI method is the more advanced version of the FUE method. The reason why we say so is the DHI method doesn’t require the surgeons to open canals on the patient’s recipient area. In the FUE method, surgeons begin the canal opening phase after extracting the donor grafts. They open tiny holes in the patient’s recipient area using small blades. 

During the canal opening phase, the donor grafts are kept in special containers. The longer the grafts are kept, the lower their chance of survival gets. Your new hair will start to grow 1 or 2 months after the shock loss period. Most likely, it will be curly and thin at first, but they get stronger in time. Before The Operation

And if you happen to choose to undergo a hair transplantation procedure, you may be curious as to what will happen next. How will the operation go, what do I have to do etc. Let’s walk through the steps one by one.

The first thing we do before each and every operation is to take pictures of your hair. These pictures are necessary for canal opening operator to determine how the implanted hair will look on you. Operators look at your natural hair formation to get an idea of how your hair looked before the loss took place.

Next step after the photography is shaving. FUE patients have to get their whole head shaved. DHI patients, on the other hand, just have to get their donor and recipient zones shaven. The rest of the head does not have to be shaved thanks to DHI pens eliminating the canal opening phase. Because in DHI, the opening of the canal and the insertion of the follicle take place at the same time.

After the required zones get a nice clean shave, we take photographs once more. This time is to track your progress after the operation. Of course, we won’t put these photographs anywhere public without your explicit permission. We just take them for medical purposes. 

The Actual Operation

Following these procedures, our medical team will escort you to your operation room. Before the operation, each patient gets a sedative pill, which helps relieve the anxiety and calms you down. At this point, depending on your demands, you may or may not get further sedation. After the optional sedation phase operators will begin anaesthetizing your donor area first. 

After anaesthetics take effect, they will then begin inflating your scalp area with a serum solution. Operators do this to increase the distance between follicles. As a result, the micromotor punch does not cause unnecessary harm to the scalp. This solution also has adrenaline added to it. Adrenaline narrows your blood veins, thus reduces capillary bleeding. This helps with fluid retention and increases overall patient comfort. 

Then following the inflation, an operator starts using the micromotor extraction device. This device generally has a punch diameter of 0.6 to 0.9 mm. The device makes tiny circular incisions around hair follicles without damaging them, and another operator harvests these follicles.

Q: My transplanted hair is falling off all of a sudden; what do I do?

The Operation In A Nutshell

Now that the groundwork for the operation is complete, the medical staff will take you in the operation room. Inside the room, the operators will give you a sedative pill which will keep your anxiety levels low and relax you throughout the operation. Some patients choose to get further sedation with the presence of an anesthesiologist. After you take your pill and you slip into your gown the team will make you lie down with face down.

Now, the next bit is the local anaesthesia. The medical team will draw a line with a marker around the back of the scalp, and they will inject anaesthesia under marked spots. After the anaesthesia is complete and the scalp skin is completely numb, the team will move on to inflation phase.

The follicles are sorted according to their quality. “How is the quality determined?” you might ask. Well, not every follicle has a single hair sticking out of it, some follicles grow out two, three, and in rare cases even four separate hairs. Your responsibilities, like work or school, may put a strain on you during this time. Also, the transplanted area will require special care for a while. For example, you cannot get the recipient area wet for at least 5 days after the operation. 

There may be some pain after the anaesthesia wears off. Though, the pain will greatly decrease, even, almost go away completely after the first night. While you can usually return to work in a week, this may not be possible for some work lines.

It is not necessary to open canals in the area where the hair will be transplanted, the existing hair follicles are not damaged during the surgery. Hair follicles grow fast after transplantation.
